Our San Jose center is the newest location to our network of medical centers, presenting patients with an innovative and advanced medical experience set within a warm and inviting atmosphere. The center is designed around the patient experience, with comfortable exam rooms, a well-equipped physical therapy space, and unique design to create a positive health care visit. Concentra provides treatment for everything from work injuries and broken bones to coughs, colds, and flu. Our medical team includes board-certified doctors, certified physical therapists, nurse practitioners, medical assistants, physician assistants, and specialist clinicians.

Specialists Services
Concentra in San Jose does our best to provide a simple, convenient health care experience for workers and their employers. With that goal in mind, we’ve partnered with medical specialists nationwide so that we can provide specialist care in our centers to employees. Called “Concentra Advanced Specialists” (CAS), our specialist network can provide fourteen different specialties, including physiatry, hand surgery, and general and specific orthopedics. Some of our centers can also provide other specialty services in addition to CAS. These can include travel health, quantitative respirator fit tests, and immigration examinations.

Concentra HUB
Concentra HUB is an integrated occupational health platform that provides 24/7 access to test results, authorizations, and reports. As soon as the data is entered into the system, users can view, print, or download the pertinent information. Concentra HUB is fully secure and requires a combined ID/password login, ensuring that each user accessed pertinent data only. Additionally, Concentra HUB offers non-injury visit results for occupational health services, client lists to verify account associations, real-time non-injury visit activity status reports, and enhanced search functionality for quick results.
Telemedicine
When an employee experiences an injury or illness at work, Concentra Telemed® can help. This easy-to-use platform can help avoid unnecessary hospital or urgent care visits, reduce lost time, and ultimately boost overall productivity.
Urgent Care Medical Center Visits
Concentra offers occupational and urgent medical care, as well as physical therapy and wellness services. We treat everything from sprains and broken bones to coughs, colds, and flu. Our clinicians include board-certified doctors, licensed physical therapists, nurse practitioners, medical assistants, physician assistants, and other medical professionals.
Telemedicine
If you have sustained minor injuries or illnesses, Concentra Telemed® is an ideal option to consider. With Concentra Telemed, you can receive timely and efficient treatment for minor sprains, strains, work-related skin conditions, and other similar injuries without having to physically visit a medical facility. Regardless of where you are or what time it is, Concentra Telemed is always there to offer you the necessary care.
