Patient forms and documents
Each of the patient forms and documents listed here will be copied and kept in the patient’s permanent file.
- Patient information form: You can print and complete this patient form in advance, or complete the copy provided at check in.
- Patient authorization form: For urgent care (a non-work-related injury or illness), you’ll need to sign a patient form that authorizes Concentra to provide services. You can print and complete this in advance or at check in. Patient authorization form (English & Spanish)
- Photo identification: Before treatment, you must show valid photo identification, which can be a driver’s license, passport, military ID, employment badge, or student identification card.
- Minor age patients: Patients under the age of 18 must be accompanied by a parent or legal guardian. If the minor is not accompanied by a parent or legal guardian, the accompanying adult must have a notarized statement signed by the parent or guardian that authorizes treatment. The parent, guardian, or adult will also need to provide photo identification, current insurance, and (as required) proof of guardianship.
- Insurance for urgent care: If your injury or illness is non-work-related and you have insurance, please bring your current insurance card.
Payment and pricing
For patients and families who plan to pay without insurance, our “no-surprise, no hidden fee” pricing is posted near the clinic entrance. All payment is due at the time of services.
- Type of payment: Concentra accepts cash, personal check, Visa®, MasterCard®, and Discover® credit cards.
- Self-pay discount: For those not paying with insurance, our Concentive$ coupons offers a discount on the cost of your visit.
- Group health insurance: We accept most health insurance.

- Q: Where can I find a list of Concentra medical center locations and operating hours?
- A: Just visit our Location Finder to find the address, hours, and contact information for any of our Concentra medical centers.
- Q: Will I need to leave personal belongings outside the collection area while I take a drug screen?
- A: You may keep your wallet with you during this procedure. Lock boxes or locking drawers are available to secure other valuables.
- Q: May I bring my children with me if I am coming for a work-related appointment?
- A: Due to the nature of certain services – for example, drug testing and physical therapy – only the patient and staff are allowed in the testing and treatment areas of our Concentra medical centers. You will need to make arrangements for children or others who might come with you.
- Q: How can I obtain the results of my drug test?
- A: Contact the employer representative who authorized the drug test.
- Q: How long will it take to get my drug test results?
- A: Drug test results typically take 24 to 48 hours, depending upon the type of test performed. The laboratory will report results to the employer representative who authorized the test.
- Q: Can prescription drugs cause a positive urine drug test result?
- A: Yes, depending on the prescription medication. Results will be evaluated with respect to proper use of valid prescription medications that could be the cause of a non-negative drug test. It is not necessary to bring any prescription medications with you for drug testing. Be sure to speak to your employer or your company’s Medical Review Officer, who can evaluate this factor before declaring your results as non-negative.
- Q: Are my drug test results confidential?
- A: Yes. Concentra follows test result information disclosure laws. We will only communicate results to the authorized employer representative who requested the test.
- Q: How can I obtain the results of my drug test?
- A: Your employer representative will receive the results of a required drug screening.
- Q: How can I obtain copies of my medical records from Concentra?
- A: Present your photo ID at the Concentra medical center where you received service and complete the Request for Medical Records form. We’ll send you your medical records in two to three weeks.
- Q: What is your policy and process for treating minors?
- A: A parent or legal guardian needs to accompany the minor. If you are not the parent or legal guardian, you will need a notarized statement signed by the parent or legal guardian authorizing you to give permission for services and billing, if appropriate.
