Thank you for choosing Concentra Primary Care for your health care needs.
Upon arriving at the center for your appointment, you will need to fill out a new patient authorization form, which is available upon check-in by our front office staff.
Please arrive at least 15 minutes before your scheduled appointment time and remember to bring the following:
- Insurance card — if you are paying with insurance, please bring your current insurance card
- Photo identification — you must show valid photo identification, which can be a driver’s license, passport, military ID, employment badge, or student identification card.
- Immunization record (if available)
- Current list of your medications
Payment and pricing
For patients and families who plan to pay without insurance, our "no-surprise, no hidden fee" pricing is clear. Payment is due at the time of services.
Type of payment: Concentra accepts cash, personal check, Visa®, MasterCard®, and Discover® credit cards.
Group health insurance: Concentra Primary Care continues to join additional health insurance plan networks. Contact your local Concentra Primary Care center for the most up-to-date list of plans accepted.
