TotalCare’s Health Improvement Program brings a comprehensive health promotion service to the workplace that supports the well, fit, and safe worker, and helps reduce employer’s overall health and productivity costs. Combining best-practice strategies to prevent cardiovascular and other chronic disease, this multi-dimensional service line is delivered seamlessly to the workforce through facilitation and coordination from Concentra and our wellness partners.

Concentra’s Health Improvement Program has five primary elements:

  • Program Consultation — serves to initially inform on the most effective delivery model, and is based on the existing workforce needs and opportunities. Workplace characteristics, employee demographics, industry type, existing benefits structure, and design are considered with a focus to maximize participation in the Health Improvement Program and enhance return on investment. Recommendations for improving the workplace environment to promote health nutritional practices and encourage physical activity are also provided.
  • Health Risk Appraisal and Biometric Screenings — helps identify and inform individuals about their own risk factors for future illness or disability. Concentra’s At Work unit facilitates the administration of a comprehensive health risk appraisal tool. Participants answer questions related to health behaviors and disease risks, and then undergo biometric screenings to obtain critical data.
  • Data Aggregation, Analysis, and Risk Stratification — reflects steps following the collection of the health risk appraisal and biometric screening information. These individualized health summaries are distributed to each participant, providing information on health risks and recommendations for lifestyle changes. Concentra provides a medical quality assurance role, including physician review of individual results. Multi-year results can be analyzed and presented so participants can track their progress over time. A corporate profile is also compiled from aggregate data that illustrates the health risks of the total workforce. This information highlights behaviors responsible for driving employer health and productivity costs.
  • Health Coaching and Ongoing Support — are the keys to the success of the Health Improvement Program. Either onsite or via telephone, health consultants evaluate each participant’s individual health profile and related risks. This component assists participants in the development of a personal action plan to improve their health statuses. Through sustained education, monitoring, and coaching, individuals begin steps toward making lifestyle changes that reduce their risks of illness, creating more productive and more fulfilling lives.
  • Targeted Lifestyle Support — will be provided to participants with specific health concerns. Depending on the needs of the workforce or workplace, these programs may be provided at the workplace by staff from onsite or At-Work units. Targeted Lifestyle Support may include: Support for weight management, improved nutrition, increased physical activity, enhanced work-life skills, and tobacco cessation. A special program aimed at reducing the prevalence of metabolic syndrome and its related heart and diabetes risks includes a focus on weight loss and emphasizes reduced caloric intake. Improved care outcomes for persons with established disease will also be attained by improving medication adherence and clinical follow-up through integrated disease management and collaboration with their health care teams.

Concentra provides a full range of health improvement services designed to optimize employee health and reduce overall costs. The Health Improvement Program can be implemented easily, with choices of where and how employee data is gathered, how employees complete their biometric screenings, and how often Targeted Lifestyle Support take place.
