Q: Where can I find a list of Medical Center locations and operating hours?
A: This Web site has a Location Finder allowing you to choose your search parameters for any Concentra Medical Centers location.
Q: What if your available locations are not where I need service?
A: The nearest Concentra Medical Center may be able to offer a provider referral, and the phone number to your nearest Concentra Medical Center can be found using our Location Finder. Your insurance carrier should be considered a helpful resource to you, as well.
Q: How can I obtain employer authorization forms, emergency and urgent care posters, wellness materials, and additional Concentra materials?
A: For your immediate need, an electronic copy of the Employer Authorization for Treatment Form is always available on this Web site. For additional Concentra materials, your local Account Representative or Center Administrator will be happy to deliver or mail additional supplies of requested items. Use our Location Finder to find the telephone number of your local Concentra Medical Center.
Q: What should employees or job candidates bring when they visit a Concentra Medical Center?
A: Employees must bring a completed Employer Authorization for Treatment Form signed by your designated company representative indicating the services to be performed at the Center. The employee or job candidate must also provide a photo ID, for example, a driver's license, work badge, or passport.
Q: May employees bring their children along with them?
A: Due to the nature of certain services – for example, drug testing and physical therapy – only the patient and staff are allowed in the testing/treatment area. Please alert your employees so they can make arrangements for children or others who might otherwise be accompanying them to the medical center.
Q: How long are the service wait times in your Centers?
A: Patients with work-related injuries or illnesses will be treated on a walk-in basis with no appointment necessary. Injuries/illnesses needing urgent care will be treated ahead of others waiting in the Center. Non-urgent injuries/illnesses will be treated within the normal flow of waiting patients to help reduce wait times.
Many of our Centers offer "fast-track" service for drug testing, so you may want to call ahead to avoid longer wait times. You may also wish to have your employees schedule an appointment for exams that require additional paperwork and testing. Telephone numbers to any Concentra Medical Center may be found using our Location Finder.
Q: How can I learn more about the testing programs for federally regulated programs such as DOT, FAA, etc.?
A: Our Web site contains (direct links) to many of the federal government's departments that regulate these types of testing. We encourage you to check out these resources.
Q: What are the qualifications of ATN, the lab used by Concentra Medical Centers?
A: ATN (Advanced Toxicology Network), providing workplace drug testing since 1997, is one of the nation's most advanced toxicology laboratories and is both SAMHSA-certified and CLIA-licensed. Performing over 8,000 immunoassay-screening tests per day, as well as more than 900 GC/MS confirmations daily, ATN is also one of the country's largest single-site drug testing laboratories.
Q: Does Concentra perform direct observation urine drug screen collections?
A: Concentra adheres to all federally regulated rules regarding direct observation in defined circumstances, including suspected adulteration.
Q: How long will it take to get Drug Test results?
A: Drug test results typically take 24 to 48 hours, depending upon the type of test performed, e.g., urine, hair, DOT, etc. If we serve only as the collection site for your drug tests, results are reported back to you directly from your selected Lab/TPA, and their result turnaround times may vary.
Q: Do you have special procedures for testing Minors?
A: A parent or legal guardian should accompany the minor. If you are not the legal guardian, you will need a notarized statement signed by a parent or signed by the legal guardian authorizing you to give permission for services and billing, if appropriate.
